Below you will find the membership application which must be filled out before you receive your first delivery.  Feel free to print it yourself and fill it out prior to our arrival or one of our drivers will happily bring you a copy to fill out.  You will only need to fill it out once and there is no membership fee!  However, in order to receive medicine from Cookie Jon's MM you must have a valid unexpired Michigan medical marijuana card and valid unexpired driver's license or state ID with picture.
